Campaign Donations
Until I am able to make a more convenient method of donating to my campaign available on this
website, you can mail me a personal check. However, please read all of the following before doing so.
Kentucky law requires us to use our best efforts to collect and report the name, address,
occupation and name of employer of individuals whose contributions exceed $100 per election cycle.
We may accept contributions from an individual totaling up to $1,000.00 per election.

Please include, with your personal check, the following information:
- Your full name.
- Your mailing address.
- Your occupation.

The name of your employer.
If you are self-employed, please list the name of the entity that apprears on your paycheck and your
line of work or the type of your business.
- Your phone number (optional).
- We would appreciate Partners of Partnerships and Members of LLC's contribute individually versus through the entity.
By making a contribution to my campaign, you are confirming that the following statements are true and accurate:
- I am not a foreign national who lacks permanent residence in the United States.
- I am not a Federal government contractor.
- This contribution is made from my own funds, and not those of another.
- This contribution is not made from the funds of a corporation or labor organization.

This contribution is made on a personal credit card or debit card for which I have the legal
obligation to pay, and is not made either on a corporate or business entity card or on the card of another person.
- I am at least eighteen years old.
